The Zanzibar health care delivery system has been divided into tertiary, secondary and primary care services; the primary health care services being spread throughout the islands (Pemba and Unguja)s. The primary health care services have been transferred to President’s Office, Regional Administration, and Local Government Special Department (PORALGSD). The distribution allows good access to primary services to 95% of the population living within or less than 5km to the nearest public health facility. Through the Chief Pharmacist Office (CPO) and the Central Medical Stores (CMS); MOHSWGEC is responsible for procurement of commodities in collaboration with the Procurement Unit. The CMS is responsible to store and distribute commodities to all public health facilities. The health procurement and logistics systems are challenged by limited qualified human resources and issues related to contract management including order processing, delivery and payment terms. All these issues impact the efficiency of the health procurement system hence compromising availability of health commodities. In November 2013, the MOHSWEGC conducted a strategic review of the national supply chain for health commodities. The review examined broader systems issues affecting the availability of commodities, the performance of the supply chain, and CMS’s ability to fulfill its responsibilities. Supply chain partners and stakeholders proposed interventions in five key areas: financing, policy, and planning; warehousing, inventory management, and distribution; system design and the LMIS; procurement and the private sector; and human resources. The review recommended CMS transformation; and implementation of the recommendations and resulted to CMS achieving a semi-autonomous status.Some additional and notable benefits of the 2013 review include storage capacity increase, and improved distribution of health commodities. This improved availability and accessibility at all times. The MOHSWGEC has continued to strengthen collaboration between alternative medical practitioners and health institutions in areas of drug monitoring and research. The cold storage system for drugs in Unguja and Pemba was also strengthened. Whilst a lot has been achieved to date, there are still some gaps that require immediate attention as noted below:<ol style="background-color: white; color: #161619; font-family: "Open Sans", sans-serif; font-size: 14px; line-height: 1.5; padding: 0px 0px 0px 15px;"><li style="background: 0px 0px; border: 0px; line-height: 1.5; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">The procurement system for health commodities and supplies is still challenged with lengthy procurement processes, poor specifications for some health commodities, inadequate and unpredictable funding for medicines and inadequate infrastructure contributing to shortages of drugs</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">The increase of demand for health services has led to increase in number of health facilities and hence it is recommended to strengthen security at HFs premises, health commodity monitoring compliance of rational drug use practices by health providers</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">Recent initiatives such as digitization of the Zanzibar health system, decentralization of the health care services and modernization of logistics system requires RGOZ to also ensure that the public health supply chain system is upgraded to meet the new expectations</li></ol>Despite the positive development of the overall Zanzibar health supply chain performance, there are still areas to improve. Based on Zanzibar Annual performance review reports of 2015/16; Zanzibar had experienced stock outs of essential commodities. There was an increase in the proportion of facilities experiencing stock out of tracer drugs from 37% in 2013/14 to 68% in 2015/16.In addition, according to the performance analysis of the Zanzibar Supply chain action plan 2017/2020, factors such as inadequate funding, inadequate quantification skills, poor planning and coordination, inadequate tracking mechanisms and tools, as well as inadequate pharmaceutical human resources at the facility level lead to poor availability of health commodities. Capacity at health facilities to use the Integrated Logistics System (ILS) remains inadequate (knowledge, time, supervision). Overall record keeping (inventory control, prescribing, dispensing) is weak which facilitates leakages of supplies to the private sector. Adherence to good storage practices varies.  Therefore, the MOHSWEGC conducts a holistic review of the health supply chain in order to fully address the observed challenges identified in the entire system. In this case the review will assess the current situation, analyse options available to address areas requiring improvement and develop strategies to strengthen the health supply chain. The Geographical health infrastructure in Zanzibar has been distributed into primary, secondary and tertiary levels of health care services. The distribution allows good access to primary services to 95% of the population living within or less than 5km to the nearest public health facility. The health supply chain in Zanzibar spans from the Central level (MOHSWGEC, Programs, CMS) to the downstream (Hospitals, PHCU+ and PHCU) over which execution of supply chain activities demands the presence of a well-trained workforce. Central Medical Stores (CMS) is responsible for procurement of commodities in collaboration with the Procurement Unit of the MOHSWGEC. The CMS is also responsible to store and distribute commodities to all public health facilities. Over time there have been efforts to improve inventory management and warehouse infrastructure at CMS, these includes the establishment of the current WIB at Unguja and building of the new warehouse in Pemba including strengthening the cold storage system. Efforts have also included implementation various logistics systems such as the of the Zanzibar Integrated Logistics System (ZILS), TB and Leprosy medicines logistics system, and the Laboratory logistics system.Based on its role, CMS is a key player in the health supply chain in Zanzibar. CMS delivers medicines, medical supplies and laboratory reagents direct to about 167 health facilities. Orders from health facilities are made electronically to CMS using the eLMIS. CMS uses mSupply as a program to run its business operations.  Whilst a lot has been achieved to date to improve warehouse and distribution, there are still a lot to unveil and provide the relevant recommendations and improvement support to ensure that there is defined standards for product selection, specification and demand planning of health commodities in Zanzibar. The MOHSWGEC is currently undertaking a holistic review of the health supply chain in Zanzibar to identify and address challenges in entire system as part of strategies to improve management of health commodities and ensuring 100% stock availability of essential medicines in all health facilities in the country. ScopeThe scope of the review includes assessing and providing recommendation on the end to end supply chain of health commodities at both MOHSWEGC, central (CMS) and health facilities. In building up recommendations for product selection, specification, and demand planning of health commodities improvement in addition to the assessment observations there will be a need to incorporate findings and recommendations from other modules like, human resource, procurement and financing and costing. The Scientist will be required to write and publish high quality scientific papers in peer reviewed and internationally recognized journals; and mentor and supervise research students to prepare reports and presentations. The Scientist will also present on progress and outcomes of the Deep Transmission research work to project and department staff and will write proposals and compete for research grants to address scientific questions in line with strategic objectives of IHI, and the public health needs.Duties and Responsibilities<ul style="text-align: left;"><li>Admin & Supervision Roles </li><li>Maintain constant liaison and engagement with community members in the study areas to ensure smooth running of project activities.</li><li>Ensure that all relevant institutional, national and international regulations regarding scientific research are adhered to during the course of all project activities.</li><li>Ensure smooth working relationship between the project (project staff) and other projects within the institute.</li><li>Supervise research interns working with the team.</li></ul>Scientific and Other Roles <ul style="text-align: left;"><li>Supervise the technical staff, volunteers and field workers working within the project</li><li>Ensure that all relevant quality control procedures and ethical guidelines are adhered to and that any deviations are properly documented and reported during all project activities and in accordance with requirements of good experimental practices</li><li>Report on progress and outcomes of the project from time to time, by way of electronic or printed periodic reports, institutional presentations and relevant scientific conferences, where feasible</li><li>Write and publish high quality scientific papers on your research work in peer reviewed and internationally recognized journals</li><li>Perform any other duties as may be assigned by the project leader</li></ul>Qualification and Experience<ul style="text-align: left;"><li>MPhil in Data Science with specialization in Financial Technology.</li><li>Working experience of 2 years.</li></ul> Skills and Competencies<ul style="text-align: left;"><li>Good writing skills</li><li>Good scientific/investigational skills</li><li>Good communication skills</li><li>Good interpersonal skills</li><li>Leadership skills and team work</li><li>Data analysis</li><li>Ability to use open-source software for data analysis (R and Python) Capability to design and execute the research protocol.</li><li>Information Technology (IT): Highly proficient in Microsoft Office Suite (Word, Excel and PowerPoint)</li><li>Programming and Statistical Analysis Tools: Proficient in R, SQL and ACL (Audit Command Language), Python, JavaScript and Solidity.</li><li>Machine Learning Principles and Techniques: Highly proficient and competent in using the following supervised and unsupervised machine learning model techniques: Regression, Decision Trees, Bagging, Random Forest, Gradient Boosting, Support Vector Machines, Neural</li><li>Networks, Cluster Analysis, Association Rules and Dimensionality Reduction Techniques (Principal Component Analysis (PCA), Multidimensional Scaling (MDS), Self- Organizing Maps (SOM) and Isometric Mapping (ISOMAP).</li><li>Analytics and Visualization Tools: Proficient with hands-on experience on using Business Intelligence and visualization platforms such as R Shiny, QlikView and Power BI.</li><li>Presentation skills.</li></ul>RemunerationAn attractive and competitive remuneration package will be offered to successful candidates as per IHI salary scales.Equal OpportunityIHI is an equal opportunity employer.
