The Zanzibar health care delivery system has been divided into tertiary, secondary and primary care services; the primary health care services being spread throughout the islands (Pemba and Unguja)s. The primary health care services have been transferred to the President's Office, Regional Administration, and Local Government Special Department (PORALGSD). The distribution allows good access to primary services to 95% of the population living within or less than 5km to the nearest public health facility. Through the Chief Pharmacist Office (CPO) and the Central Medical Stores (CMS); MOHSWGEC is responsible for procurement of commodities in collaboration with the Procurement Unit. The CMS is responsible for storing and distributing health commodities to all public health facilities. The health procurement and logistics systems are challenged by limited qualified human resources and issues related to contract management including order processing, delivery and payment terms. All these issues impact the efficiency of the health procurement system hence compromising availability of health commodities. In November 2013, the MOHSWEGC conducted a strategic review of the national supply chain for health commodities. The review examined broader systems issues affecting the availability of commodities, the performance of the supply chain, and CMS’s ability to fulfill its responsibilities. Supply chain partners and stakeholders proposed interventions in five key areas: financing, policy, and planning; warehousing, inventory management, and distribution; system design and the LMIS; procurement and the private sector; and human resources. The review recommended CMS transformation; and implementation of the recommendations and resulted in CMS achieving a semi-autonomous status.Some additional and notable benefits of the 2013 review include storage capacity increase, and improved distribution of health commodities. This improved availability and accessibility at all times. The MOHSWGEC has continued to strengthen collaboration between alternative medical practitioners and health institutions in areas of drug monitoring and research. The cold storage system for drugs in Unguja and Pemba was also strengthened. Whilst a lot has been achieved to date, there are still some gaps that require immediate attention as noted below:The procurement system for health commodities and supplies is still challenged with lengthy procurement processes, poor specifications for some health commodities, inadequate and unpredictable funding for medicines and inadequate infrastructure contributing to shortages of drugsThe increase of demand for health services has led to increase in number of health facilities and hence it is recommended to strengthen security at HFs premises, health commodity monitoring compliance of rational drug use practices by health providersRecent initiatives such as digitization of the Zanzibar health system, decentralization of the health care services and modernization of logistics system requires RGOZ to also ensure that the public health supply chain system is upgraded to meet the new expectationsDespite the positive development of the overall Zanzibar health supply chain performance, there are still areas to improve. Based on Zanzibar Annual performance review reports of 2015/16; Zanzibar had experienced stock outs of essential commodities. There was an increase in the proportion of facilities experiencing stock out of tracer drugs from 37% in 2013/14 to 68% in 2015/16.In addition, according to the performance analysis of the Zanzibar Supply chain action plan 2017/2020, factors such as inadequate funding, poor planning and coordination, inadequate tracking mechanisms and tools, as well as inadequate pharmaceutical human resources at the facility level lead to poor availability of health commodities. Capacity at health facilities to use the Integrated Logistics System (ILS) remains inadequate (knowledge, time, supervision). Overall record keeping (inventory control, prescribing, dispensing) is weak which facilitates leakages of supplies to the private sector. Adherence to good storage practices varies. Therefore, the MOHSWEGC conducts a holistic review of the health supply chain in order to fully address the observed challenges identified in the entire system. In this case the review will assess the current situation, analyse options available to address areas requiring improvement and develop strategies to strengthen the health supply chain. The Geographical health infrastructure in Zanzibar has been distributed into primary, secondary and tertiary levels of health care services. The distribution allows good access to primary services to 95% of the population living within or less than 5km to the nearest public health facility. The health supply chain in Zanzibar spans from the Central level (MOHSWGEC, Programs, CMS) to the downstream (Hospitals, PHCU+ and PHCU) over which execution of supply chain activities demands the presence of a well-trained workforce. Central Medical Stores (CMS) is responsible for procurement of commodities in collaboration with the Procurement Unit of the MOHSWGEC. The CMS is also responsible to store and distribute commodities to all public health facilities. Over time there have been efforts to improve inventory management and warehouse infrastructure at CMS, these includes the establishment of the current WIB at Unguja and building of the new warehouse in Pemba including strengthening the cold storage system. Efforts have also included implementation various logistics systems such as the of the Zanzibar Integrated Logistics System (ZILS), TB and Leprosy medicines logistics system, and the Laboratory logistics system.Based on its role, CMS is a key player in the health supply chain in Zanzibar. CMS delivers medicines, medical supplies and laboratory reagents direct to about 167 health facilities. Orders from health facilities are made electronically to CMS using the eLMIS. CMS uses mSupply as a program to run its business operations.  Whilst a lot has been achieved to date to improve warehouse and distribution, there are still a lot to unveil and provide the relevant recommendations and improvement support to ensure that there is defined standards for product selection, specification and demand planning of health commodities in Zanzibar. The MOHSWGEC is currently undertaking a holistic review of the health supply chain in Zanzibar to identify and address challenges in entire system as part of strategies to improve management of health commodities and ensuring 100% stock availability of essential medicines in all health facilities in the country. ScopeThe scope of the review includes assessing and providing recommendation on the end to end supply chain of health commodities at both MOHSWEGC, central (CMS) and health facilities. Below eight areas of Supply chain have been identified for the review:<ul style="text-align: left;"><li>Products specifications, Selection, and demand planning,</li><li>Costing, funding, and financing strategies </li><li>Procurement and contract management</li><li>Warehousing, storage and spaces management, distribution, and transportation network at CMS</li><li>Inventory management (inventory analysis – SKUs (Stock keeping units), weight/volume, stock location, stock control policies, throughput analysis)</li><li>Information management systems </li><li>Human resource capacity for supply chain management</li><li>Supply chain oversight, governance, and accountability</li></ul>The goal is to have a robust supply chain system that is both flexible and adaptable to frequent changes and evolving demands of the health sector while ensuring that the health commodities reach clients timely, cost effectively and with guaranteed quality.Purpose StatementThe purpose of this scope is to assess the current health commodities product selection, specification and demand planning of health commodities process and recommend the needed improvements for gaining efficiencies. In building up recommendations for product selection, specification, and demand planning of health commodities improvement in addition to the assessment observations there will be a need to incorporate findings and recommendations from other modules like, human resource, procurement and financing and costing. The scope of this module aims at analyzing gaps in health commodities (Pharmaceuticals, laboratory, and diagnostics) in three aspects of product selection, specifications, and demand planning to create standardized list of items as per level of care. Objectives<ul style="text-align: left;"><li>Determine the process of product selection, specification, and demand planning of health commodities (essential medicines, diagnostics, medical reagents, and equipment),</li><li>Determine the gaps on areas of product selection, specifications, and demand planning.</li><li>Identify barriers hindering adherence to the pre-set standards for product selection, specification, and demand planning,</li><li>Recommend measures for efficient product selection, specification, and demand planning of health commodities in Zanzibar.</li></ul>Tasks to be performed. For effective review, the review will be required to complete the following tasks.<ul style="text-align: left;"><li>Review preparations and logistics: The Consultant will be responsible for development of the action plan and related schedules for review and assessment to be conducted. </li><li>Conduct the document review: The Consultant will be required to conduct the document review on: Demand planning assessment reports, product selection, specifications, and demand planning reports for essential commodities, laboratory commodities, and vertical program quantification reports. These include social security contributions, Workers Compensation Fund contribution, Staff HELSB Loans Board deductions;</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; list-style: none; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">Taxation: Ensuring that full and accurate company tax reports are prepared and submitted to the TRA on or prior to TRA deadlines and that tax payments are made to TRA on or prior to the legally prescribed time frames. This includes such taxes such as PAYE, SDL, VAT, Withholding taxes and any other tax types;</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; list-style: none; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">Imprest Accounts: Ensure that all imprest accounts are accurately and well recorded, timely retired and correctly reconciled;</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; list-style: none; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">Pre-payment Checks: Check that procedures and processes are followed prior to finalisation of payments in accordance with company standards;</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; list-style: none; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">Bank Accounts: Ensure that bank reconciliations are properly done by the fifth working day of the following month and reported to the Line Manager;</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; list-style: none; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">Training and Support: Train new employees on the management of EDI Global funds & assets and the maintenance of other financial standards;</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; list-style: none; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">Internal and External Auditing: Prior to and during audits, prepare all required documentation and cooperate with the external auditors throughout the audit exercise;</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; list-style: none; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">Expenditure Forecast: Ensure that the monthly funds request (MFR) is accurately prepared and implemented timely and updates to the same are made as necessary;</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; list-style: none; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">Insurance: Checking that all office equipment, staff and vehicles are insured and insurance claims for lost/stolen office equipment are processed timely;</li><li style="background: 0px 0px; border: 0px; line-height: 1.5; list-style: none; margin: 5px 0px; outline: 0px; padding: 0px; vertical-align: baseline;">Account Receivable Management and Invoicing: In liaison with Project Team Leaders; ensure that invoices are submitted to clients in a timely manner.
